R12: Purchase Order Form Does Not Open And FRM-40735 Is Occurred

Symptoms
On : 12.1.3/12.2 version, Purchase Order
When attempting to opening the Purchase order, the following error occurs.
ERROR
-----------------------
FRM-40735: PRE-FORM trigger raised unhandled exception ORA-06508
The Purchase order form does not open. (The 'Buyer Work Center' works fine.)
STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. Responsibility = Purchasing
2. Navigation = Purchase Orders > Purchase Orders
Changes
The problem occurred after an initial set up of Purchasing, Inventory Management and I-Procurement in a R12.1.3 environment.
