Collection Plan Entries Stuck With Status = 1 In QA_RESULTS_V (Doc ID 2045641.1)

Recently a new collection plan was created with transactions trigger. Use the "Enter Quality Results" form or the "Update Quality Results" form from Service Request form, a new record entered or updated and saved. The transaction appears to have saved (pop-up confirmation with transaction saved message), however, when querying the database views, the record remains in QA_RESULTS_V with status = 1 and does not get transferred to the actual base view of the collection plan. Because of this, none of the data can be seen from "Update Quality Results" form and the valuable data is being lost. Why does this happen?
