Symptoms
When attempting to add attachments to the Supplier and/or Purchasing documents the following error occurs.

oracle.apps.fnd.framework.OAException: oracle.jbo.DMLException:
JBO-26041: Failed to post data to database during "Insert" SQL Statement "INSERT INTO
FND_LOBS(FILE_ID,FILE_NAME,FILE_CONTENT_TYPE,FILE_DATA,FILE_FORMAT)
VALUES (:1,:2,:3,:4,:5)".
