E-Business Suite 12.1.3 Concurrent Processsing, Concurrent Manager Issues

Unable to start the concurrent managers - FNDSM Service Manager could not initialize.

After applying several patches the service manager won't start, only the internal manager starts.

The below error message appears in the internal manager log:

Service Manager FNDSM_<nodename>_<db name> could not initialize. The Service Manager has been unable to verify its environment and create its log file.
shut_down_service called for node <nodename>
clear_service_context called for node <nodename>
An error occured in client-side routine afpsmcsm for Service Manager <nodename>_<db name>. The routine returned code 127.

Patches were recently installed, and/or APPLCSF environment variable changes were recently implemented (unset APPLCSF).
