Symptoms

On : 12.2.5 version, Installation

While applying the EBS Release 12.2.5 Patch 19676458 to upgrade from R12.2.4 to R12.2.5, one of the worker failed to execute csfhook.ldt job, from the worker log i see below error.

Uploading from the data file /<PATH>/appl/csf/12.0.0/patch/115/import/US/csfhook.ldt
Altering database NLS_LANGUAGE environment to AMERICAN
Dumping from LCT/LDT files (/<PATH>/appl/jtf/12.0.0/patch/115/import/jtfhook.lct(120.4), /<PATH>/appl/csf/12.0.0/patch/115/import/US/csfhook.ldt) to staging tables
Dumping LCT file /<PATH>/appl/jtf/12.0.0/patch/115/import/jtfhook.lct(120.4) into FND_SEED_STAGE_CONFIG
Dumping LDT file /<PATH>/appl/csf/12.0.0/patch/115/import/US/csfhook.ldt into FND_SEED_STAGE_ENTITY
Dumped the batch (JTF_HOOKS_DATA 10084 , JTF_HOOKS_DATA 12366 ) into FND_SEED_STAGE_ENTITY
Uploading from staging tables
 Error loading seed data for JTF_HOOKS_DATA: HOOK_ID = 10084, ORA-01422: exact fetch returns more than requested number of rows


STEPS

The issue can be reproduced at will with the following steps:
1. Apply patch 19676458

The same error was also encountered for HOOK_ID=2261
