TeleService 12.2.4 - Issue Compiling Form CSXSRISR.fmx During Patch Installation (Doc ID 2078545.1)

Symptoms

When installing the latest Patch Wizard merged patch set, it attempts to recompile forms.
During forms recompilation there are errors.

Errors:

The following Oracle Forms objects did not generate successfully:
