R12: AP: Why Employee Supplier has Missing Address?

Why after upgrade to R12, employee supplier has address missing in multiple places:

1) Employee type supplier checks do not have address printed.
2) The employee addresses are missing in the Supplier Master. There is a site name but no address details.
3) Also, if you go to create a new address there is no fields to add address1, city, state, etc.

How do we fix this ?
