Errors Saving Online Receipts Post Upgrading Database - rvtptcontrol failed: RVTPT-054: ORA-01086: ORA-07445: APP-FND-00204 (Doc ID 2105480.1)

Symptoms

On 12.1.3 instance, Online Receipts Fail with the following error messages:

rvtptcontrol failed:
RVTPT-054: ORA-01086: savepoint 'START_TRX' never established in this session or
Cause: A SQL error has occurred in RVTPT. The SQL error is &LSQL_ERR.

RVTII-060: Subroutine INV_TXN_MANAGER_PUB.process_Transactions() returned error

Or

APP-FND-00204: Concurrent Manager encountered an error while running the spawned concurrent program Receiving Transaction Manager - RCVOLTM for your concurrent request xxx. TM-TIMEOUT.

Trace file shows several instances of error below:

ORA-07445: exception encountered: core dump [evaopn3()+107] [SIGSEGV] [ADDR:0x0] [PC:0xCBF74CB] [Address not mapped to object]

Steps:
1. Ensure profile option RCV: Processing Mode is set as 'ONLINE'
2. Navigate to Inventory > Transactions > Receiving > Receipts
3. Query expected receipts for the document in question
4. Enter Receipts details and save
5. Error occurs
