R12: OIE: Internet Expense Report Status Is Pending Manager Approval Even After Approved By Manager

Symptoms
On : 12.1.X version, Approval Workflow & Builder Issues
For some expense reports, even after the Manager has approved it , the status is still Pending Manager Approval.
It is expected to be listed as approved. Due to this issue, employee are not reimbursed and escalations occur in the organization.
The issue can be reproduced at will with the following steps:
1. Internet Expenses Audit screen, review sample expense report, notice it was approved by "XXXXX" but the status is 'Pending Manager Approval'
