E-Business Suite 12.2 Concurrent Processing, Request Processing issues

After a recent upgrade from 12.0 to 12.2 the form FNDRSNRUN.fmb is very slow.

The following navigation results in a performance issue for all users:

From Tool Bar View > Request > Submit a New Single Request > Single Request > OK

It takes too long to open 'Submit Request' form.
