Workflow Errors and Real Time Customer Assignment not Working in R12

On : 12.2.4 version, Territory Assignment

We are getting the following error when creating a customer and we have noticed that the real time assignment is not working correctly

This error is appearing after activating the Subscriptions for TAP realtime assignment as per Doc ID 554004.1

The issue can be reproduced at will with the following steps:

1) Navigate Sales Manager Responsibility
2) choose Customer function
3) Create a test customer
4) customer creation succeeded, but from sysadmin I can see workflow is in error.

5) Also note that the correct sales rep is not assigned to the customer record
