Errors in generating OKLCSACT.fmb and OKLCSHDR.fmb

Symptoms
When applying applying a patch, or regenerating form files using adadmin, the following errors are hit:

The following Oracle Forms objects did not generate successfully:
okl forms/FRC OKLCSACT.fmx
okl forms/FRC OKLCSHDR.fmx
An error occurred while generating Oracle Forms files.
ERROR generating form "forms/FRC/OKLCSACT.fmx" from input file
/<Dir>/OKLCSACT.fmb

FRM-30041: Position of item places it off of canvas.
Item: SEQ_TYPE_LAST

The issue can be reproduced at will with the following steps:
Apply a patch with OKLCSACT.fmb or OKLCSHDR.fmb in any language
OR
Regenerate OKL form files using adadmin.
