Running ADADMIN or ADOP After Database Upgrade Fails With Error While Running Adtasktim.sql (Doc ID 2225088.1)

Symptoms

On : 12.2.4 version,

Unable to Login EBS home page after Database upgrade from 11.2.0.3.0 to 12.1.0.2.0

Running either adadmin or adop pathces after db upgrade, fails with below error:
 
ERROR

An error occurred while Running adtasktim.sql.

And 'apps' user locks repeatedly.
