Unable To Login After April 2016 CPU Patch And Encountering Error FND_STATE_LOSS_ERROR (Doc ID 2227999.1)

Symptoms

In 12.1.3, users are unable to login using SSO and local login is failing after April 2016 CPU

Encountering the following error :
