Symptoms

When attempting to access the Life Event Reasons form, the following error occurs:

ERROR
Invalid argument to HR_form.define: P_blockname='LEE'

STEPS

The issue can be reproduced at will with the following steps:
1. Go to responsibility US HRMS Manager.
2. Navigate to Total Compensation > General Definitions > Additional Setup > Life Event Reasons.
Error occurs.


Changes

 Attempted solution as described in My Oracle Support Note 1517593.1: Opening Life Event Reason Form, Errors: FRM-40735 PRE-FORM trigger raised unhandled exception ORA-20002, but the issue still occurs.
