Resubmit Fails After RUP9

On : 12.1 HRMS RUP9 version, Workflow,Approval,Common Funct

Resubmit by a role with multiple users fail after RUP9 and gives below error:

3123: Duplicate attribute 'FORWARD_FROM_ROLE_NAME' could not be created.


Steps to Reproduce:
The issue can be reproduced at will with the following steps:

1. Go to Employee Self Service.
2. Create an Extra Information and submit.
3. Login as one of the approver from the role.
4. Open the notification and click on Update Action link.
5. Make changes and submit. Now check the status from Status Monitor, the transaction is errored.

The issue has the following business impact:
Due to this issue, users cannot complete the transaction.
