Form Personalization Not Visible Anymore (Doc ID 2288674.1)

Goal

In the Shipping Transaction Window customers have a Form Personalization (FP) for orders that will be shipped directly to the End Customer. There are 2 FPs: XXX One Time Shipment and XXX One Time Shipment on Pick Release. Although the Form Personalizations are active, users are unable see them anymore, they are not visible. How can users see them again. I searched for them Help -> Diagnostics -> Custom Code -> Personalize and they don't appear.
