Symptoms
On : 12.1.3 version, Debrief / Field Service Report
When attempting to to save a debrief, the following error occurs.

FRM-40735: PRE-INSERT trigger raised unhandled exception ORA-04062
or
FRM-40735: WHEN-VALIDATE-RECORD trigger raised unhandled exception ORA-04062
STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. Create a Debrief for a task
2. Enter the corresponding debrief lines
3. Try to save and the reported error occurs.
