DFF Not Displaying Additional Columns From Value Set (Doc ID 2355590.1)

Symptoms

User created a DFF (descriptive flexfield) in one instance (instance one) and then in another instance (instance two). The setups look to be the exact same. Instance one is showing 3 DFF columns, and instance two is only showing one DFF column. 

Why is instance two only showing one DFF column?
