R12: Incorrect and Unrelated File Attachments Added in Email Notification and Notification Worklist

R12: Incorrect and Unrelated File Attachments Added in Email Notification and Notification Worklist in "View Document Attachment"
Steps to Reproduce the issue :
1. Create a purchase order then add attachments with these types i.e: 'short text ,long text and File'
2. Submit for approval
3. PO goes to next approver
4. Approver opens the notification and then click on "View Document Attachments"
5. Or approver receive email notification
6. Notification shows 4 attachments - 3 above attachments plus another attachment example 'Scan0009.tif' that is not related to the PO uploaded attachments
Note: The issue occurs only when uploading attachments with 'Short text' or 'Long text' type along with the 'File' type attachments ,Issue working fine if just uploading 'File' type attachments.
