R12 E-Business Suite Applications Manager Patch Wizard Concurrent Request Fails With Error 'WARNING: The My Oracle Support Credentials have not been set up'

E-Business Suite R12 Applications Manager, Patch Management Features
Users are unable to get Patch Wizard to run the Recommended Patches Request Set in Oracle Applications Manager (OAM).
The users confirm they have valid Metalink credentials setup, however the Recommended Patches Request Set fails with the below error:

Customer has also applied the latest <Patch 17270136>.
Notice that Concurrent Requests in OAM Patch Wizard are being submitted as SYSADMIN, not as the user logged in.
The issue can be reproduced at will with the following steps:
1. Use the Patch Wizard to run Recommended Patches Request Set.
