Symptoms
On : 12.2.7 version when attempting to approve Standard POs or Blanket Agreements the workflow does not initiate properly and error occurs An error notification is sent to user with this in the notification body
The PO remains incomplete status and no wf_item_type or wf_item_key is populated in po headers table

Error: ORA-06502: PL/SQL: numeric or value error: character string buffer too small
This issue is occuring on UAT and testing has stopped on UAT because of the issue. It is an upgraded 11.5.10 to 12.2.7 system.
On three other environments previously this upgrade was done and this error did NOT occur on those.
STEPS

The issue can be reproduced at will with the following steps:
1. Upgrade from 11i to 12.2.7
2. Test approval of POs
3. Error occurs
