Journals Not Getting Approved Through AME: No Approver Found / ORA-20001: Possible Loop in A Chain of Authority (Doc ID 2426150.1)

Symptoms

For some GL journals, attempting to approve the journal fails as no approver found.
The issue is common with journals having more number of lines and amount is huge but lesser than Approval Limit.


ERROR:
Workflow:
NOTIFY_PREPARER_NO_APPROVER
No approver was found to approve your journal batch <name>

FND Log shows:
GLBATCH:5503556*256624:Get_Next_Approver: 002-503556 ORA-20001: The AME engine seems to have found a loop in a chain of authority, because it tried to add more than 2500 chain-of-authority approvers to the transaction's approver list.  Please check the authority hierarchies for this transaction.

STEPS to REPRODUCE:
The issue can be reproduced at will with the following steps:
1. Gl Responsibility > Journals > Enter
2. Forward to approver
3. Observe the batch goes into approval error
