On : 12.1.3 Create Service request
When creating a service request in the SR form the following error occurs when saving the SR.

Following error is given when tyring to save SR:
ORA-01422: exact fetch returns more than requested number of rows in Package CS_ServiceRequest_PVT Procedure Create_SR_Validation

The issue can be reproduced at will with the following steps:
1. create an SR for an item
2. save.
