Rarely Concurrent Programs Complete in Error "Concurrent Program Returned No Reason For Failure"

Symptoms
Once or twice per month, some concurrent requests complete in an error "Concurrent Program Returned No Reason For Failure".
You can see the message in "COMPLETION_TEXT" in fnd_concurrent_requests.
We found a concurrent manager which processed the request died in the following error.
