The issue can be reproduced at will with the following steps:
1. Apply the Patch# 28271023 then can see the below issue

The following Oracle Forms objects did not generate successfully:
ja forms/US JAINADRP.fmx
ja forms/US JAINRCLM.fmx
ja forms/US JAINTXPP.fmx
ja forms/US JAINTXOC.fmx
ja forms/US JAINTXRP.fmx
An error occurred while generating Oracle Forms files.

BUSINESS IMPACT
