Missing Equipment Search Fields In OTM 6.4.3 After Upgrade

Symptoms
Instance is upgraded from 6.1.5 to 6.4.3
Custom Equipment Screen Set moved from 6.1.5 to 6.4.3 does not show up search parameters Reference Qualifier 2, Reference Number 2, Reference Qualifier 3, Reference Number 3
that were defined as part of custom screen set in 6.1.5. These fields are also not coming when add field button is clicked as well

The issue can be reproduced at will with the following steps:
1. Navigate to Equipment Screen (Customized)
2. Look for the search fields - Reference Qualifier 2, Reference Number 2, Reference Qualifier 3, Reference Number 3
