Attachments File Upload Screen Returns Unexpected Error (Doc ID 2510011.1)

Symptoms

When attempting to add an E-business Suite (EBS) attachment on any form (i.e. Invoices, Purchase Order, Purchase Requisition, etc.) the following error message occurs:

"Error Page
 You have encountered an unexpected error. Please contact your System Administration for assistance."

However, when replicating the event and capturing an Oracle Application Framework (OAF) stack trace file as per <Note 562796.1> 'How to Diagnose 'You Have Encountered an Unexpected Error' and/or <Note 297176.1> 'Error Page : You have encountered an unexpected error. Please contact the System Administrator for assistance' the following additional error message is revealed:
