Form Personalisation Issue - Personalization Created to Display Message, but Message Displays Multiple Times

ACTUAL BEHAVIOR

Problems with Personalization on Recipe Details form.
Customer created Personalization to display a message upon Save (Trigger Event is When-Validate-Record) when certain conditions are met, based on a sql query. The message is displayed successfully, but when clicking OK, the message repeats, and keeps repeating indefinitely.
Following note 2113090.1, and applying patch 20632415:R12.FND.B, did not resolve the issue.
EXPECTED BEHAVIOR

Expect the error to display once, the recipe to not be saved, and all functions to work normally after this.
STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. Create Personalization on Recipe Details to give error when trying to save a recipe when a query returns data showing that a recipe with similar data already exists (in this case, it was specific flexfield data)
2. Create one recipe with specific data in the flexfields, then try to create a second recipe with the same flexfield data
