Symptoms

When attempting to create EAM work orders, the following error occurs. The issue exists in Forms (CORE) and Self Service (OAF).

ERROR

The Work Orders form (CORE) error:
  FRM-40735: ON-INSERT trigger raised unhandled exception ORA-01403

The Create Work Order page (OAF) error:
  You have encountered an unexpected error. Pleas contact your system administrator for assistance.

STEPS

The issue can be reproduced at will with the following steps:
1. Responsibility: Enterprise Asset Management.
2. Go to Work Orders form.
3. Try to create a new work order.
4. Receive the above error.
5. Also, unexpected error occurs in OAF page when trying to create work orders.
