Post SSL Offloading Setup For EBS URL, Forms Are Not Launching, Error FormsLauncher.class Not Found (Doc ID 2606755.1)

Symptoms

The environment was setup to use SSL offloading configured with Load -balancer and encountered the following error within forms;
