OAM Forms Session Monitoring Is Not Displaying All Sessions (Doc ID 2621777.1)

Symptoms

The Forms session monitoring page in Oracle Applications Manager (OAM) dashboard is not displaying all of the active forms sessions.

STEPS
---------
The issue can be reproduced at will with the following steps:

1. Using your Application user, login to Oracle E-Business Suite (EBS), and from the Home Page, click on any form.
2. Navigate to: OAM > Site Map > Monitoring > Under "Current Activity" click "Form Sessions", and confirm an entry exists for this new form.
3. Close out the form (do NOT finish the Forms session), refresh and check OAM again, now there should be no entry with the closed form.
4. Open another form, and check OAM again (refresh). You should see a new entry for this Form.
5. Test other users:
    a. Have another user login to EBS, and open a form.
    b. Refresh and check OAM, you should NOT see any new entries for this new user's form.
    c. Do steps 5.a and 5.b for a few users and you would expect similar results.
