Symptoms

See two records in action history when PARs are submitted for approval.    The time stamp for both is the same and the sequence 1 is Submit and Sequence 2 (duplicate) has null action code.

See a submit and only actions for future pending approvals

STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. Create PAR
2. Submit PAR for approval
