Unable to add expenditures items to existing pre-approved batch.

ORA-01403: no data found

FRM-40735: ON-INSERT trigger raised unhandled exception ORA-01400.

Steps:

1. Create new Pre-Approved batch.
2. Navigate to Review form
3. Query batch
4. Press Open button
5. Press Expenditures button
6. Enter New Expenditure line
7. Enter expenditure item details
8. Save - error

However if you query the batch via navigating Expenditures -> Pre Approved
Batches -> Enter you can add a new expenditure without error.
