Symptoms

 When attempting to access the Activity Audit form, the following error occurs.

404 The page was not found


STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. Responsibility:  Maintenance super user
2. Navigation: ECC home->Administration->Activity Audit
3. See error above.
