R12: AP: After Applying Patch Oracle 1099 Reports Are Failing With Errors ORA-7445[kkqsFindOpn()+37], ORA-24324, REP-0069, REP-57054 And REP-300

Symptoms
After DB upgrade by RDBMS <Patch 12834800>, All 1099 reports were failing with ORA-24324: service handle not initialized
Then, Application Patch 30273541:R12.AP.B was applied and ALL the 1099 reports started working except for "1099 Forms - Comma Delimited Format" report - APXT7C99.rdf.
The Report log file shows the following errors:
ORA-24324: service handle not initialized

REP-0069: Internal error
REP-57054: In-process job terminated:Terminated with error:
REP-300: service handle not initialized

The report executable, file APXT7C99.rdf was not upgraded by the application of Patch 30273541:R12.AP.B
A SQL*Trace file of the execution of the 1099 Comma Delimited Report - APXT7C99.rdf shows Error ORA-7445 occurring. Trace file shows the following:
