Error when creating Tax service mappings

When trying to create Service Mappings in R12 get an error

STEPS

The issue can be reproduced at will with the following steps:
1. Tax manager > Tax configuration > Tax regimes > Query tax regime
2. click on update continue to service subscription
3. Click on Create and error

or

Navigation : Tax managers > Provider Services > Service Mappings > Click on create button and getting the error.
