Distinct Coloring On WEB ADI Template Is Removed After Upgrade to 12.2.X

Symptoms
After upgrade from R12.1.3 to R12.2.10, the WebADI Templates have lost its distinct coloring for headers, required fields and so on.
Templates are now white with a faint grey color.
Steps to reproduce

The issue can be reproduced at will with the following steps:
From Oracle General Ledger Responsibility
1. Navigate to Journal > Launch Journal Wizard
2. Complete screens as prompt
3. Create Document
4. New form shows File creates to Excel 2007 viewer, select Create and then Open
5. Find file created in excel with white and light grey formatting
