Appraisal is not created for few employees who are part of the Performance Plan

After process “Run Mass Appraisal Creation” is submitted and completed successfully, notice that
 appraisee is unable to view/access appraisal, and the appraisal is also not with appraiser.

Error from Mass Appraisal Creation concurrent log is

The issue can be reproduced at will with the following steps:
1. From HR Professional V.4 responsibility create a new Plan with objectives and appraisal and Eligibility Profile is used for appraisals
2. Plan is published and scorecards are created
3. On appraisal task start date, run Mass Appraisal Creation
4. Notice that appraisal is not created for some employees and below error is printed in the log
Not eligible. Appraisal creation skipped for xx_employee_name
