Confirmation message box is appearing out of requisition page layout after adding attachments to requisition

EXPECTED BEHAVIOR
-----------------------
Confirmation message box should appear in the middle of the page layout

STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. Login to iProcurement 
2. Create Catalog / Non-Catalog requisition
3. Checkout
4. Add attachment at Purchase Requisition (PR) line and Apply.
5. Confirmation message box is appearing out of requisition page layout
