When attempting to Implement a Phase change of an Item, the following error occurs:

ERROR

A pending phase change already exists for item &ITEM_NUMBER in organization &ORG_NAME.  Please implement/cancel the pending phase change for the item before creating new phase changes.

STEPS
---------
The issue can be reproduced at will with the following steps:
1. Go to responsibility: Development Manager
2. Go to Simple Item Simple Search
3. Enable the Diagnostics in Settings option at Exception Level.
4. Now, Search for Item -> Click the item hyper link -> Click the Lifecycle -> Click on Change Phase.
5. Choose Promote and Apply. Post that, go to Pending Changes -> Click on Implement
6. OA Page will now show the mentioned error.
