IProcurement Not Allowing More Than 2 Line Items To Be Added To Shopping Cart Error:FND_RECORD_CHANGED_ERROR Record Contains Stale Data (Doc ID 2884781.1)

Symptoms

iProcurement not allowing more than 2 line items to be added to Shopping Cart
 
The issue can be reproduced at will with the following steps:
1. Navigate to iProcurement Responsiblity -> Non-Catalog Request (t).
2 . Enter one line details
3. Add to Cart
can see Item added successfully
4. Enter another line details
  Can see in the shopping cart 2 lines
3. Add a 3rd line
  Can see item added successfully
  however not displayed in the shopping cart
4. Select View Cart and checkout
5. Can see the 3 items in the shopping cart
  However cannot save or submit due to an error:
Unable to perform transaction on the record.
Cause the record contains stale data. The record has been modified by another user
Action: Cancel the transaction and re-query
.
If limit is only 2 lines no error and able to continue.

 6. Can see additional detail in FND log : icx.por.req.server.RequisitionAMImpl  Fatal OAException:oracle.apps.fnd.framework.OAException: Application: FND, Message Name: FND_RECORD_CHANGED_ERROR
