The "A draft Standard Purchase Order has been created for items on your Purchase Requisition" Notification Opens With Error After Adding Requisition Line to Existing Purchase Order (Doc ID 2910376.1)

Symptoms

When attempting to use the notification feature to notify Requester when a Requisition line is placed on a Purchase Order (PO), an error occurs when opening the notification.  

The notification with Subject "A draft Standard Purchase Order has been created for items on your Purchase Requisition NNNN and is awaiting approval"  is generated to the requester user but when it is opened it shows error

   ERROR
   You have encountered an unexpected error. Please contact your System Administrator for assistance.

In the debug log see this more detailed error

  “ORA-01422: exact fetch returns more than requested number of rows”, “ORA-06512: at APPS.PO_COMMUNICATION_PVT", line 10050” and “ORA-06512: at "APPS.PO_COMMUNICATION_PVT", line 9885”

STEPS
The issue can be reproduced at will with the following steps:
1. In iProcurement, create Requisition and check “Notify me with status updates for my Purchase Order”
2. Approve this Requisition
3. Go to Purchasing responsibility and navigate to Autocreate form and create this Requisition to a Purchase Order  in this form
4. Create new Requisition that has same item /data
5. Go to Purchasing responsibility and navigate to Autocreate form .  Query the new Requisition in the Search and choose Add option to add to Line 1 of the original PO
6. This PO Line has two shipments, one with Promise Date and one with null promise date
7. Once added to the PO the notification to notify the Requester is automatically launched via the PO Approval workflow
8. User clicks Subject link to open the notification which has subject "A draft Standard Purchase Order has been created for items on your Purchase Requisition NNNN and is awaiting approval" error occurs
