HRMS Document Of Records Are Missing After Approval

Symptoms
Actual Behavior
When submitting Documents of Record (DOR) from the 'Documents of Record for person' Self Service page
After approval of DOR,
- Approval Notification Email is sent
- But the DOR never appears in Documents of Record page
- In Workflow Administrator panel - Transaction still shows as Active
Expected Behavior:
Workflow should complete - as approvals have been carried out
Steps to Reproduce:
The issue can be reproduced at will with the following steps:
1. Navigate to Employee Self Service > Documents of Record
2. Submit DOR - sends into workflow for approval
3. Confirm manager received the email, and has approved workflow
4. Navigate back to Documents of Record page
- Item is NOT visible
- In Workflow Administrator panel - Transaction still shows as Active
