Not Able To Launch / Open the Approval Assignments Form (Doc ID 2928733.1)

When trying to launch / open the Approval Assignments form, nothing happens. There is no error but the form in not opening.

Steps to reproduce

1. Log into Purchasing Super User responsibility
2. Select Approval Assignments
3. Form is not opening
