Symptoms
On : 12.2.10 version, Item Issues
When an item is assigned to an inventory org, there is no issue. But when trying to remove the assignment from the Organization, the following error occurs:
ERROR

APP-FND-01206 This record already exists. You entered a duplicate value or sequence of values that must be unique for every record
STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. Inventory Responsibility
2. Items>>Master Items>Go to Org Assignment tab
3. Attempt to uncheck Organization assignment>>See error
