Symptoms
We have Allow Dynamic Inserts enabled as for the Accounting GL Flexfield structure. Yet while defining the Bank Statement Cashflow Mappings we receive an error.
APP-FND-00756: Cannot Find combination CCID= -1 CODE =GL# on Bank Statement Cashflow mapping rules while trying to add the GL Account Number
STEPS

The issue can be reproduced at will with the following steps:
1. Log into the Cash Management responsibility
2. Navigate to Setup / Banks / Bank Statement Cashflow Mappings
3. Enter "Transaction Code" 469 and query for Search String like '%PAYROLL%
4. Error 'APP-FND-00756" Cannot find combination CCID=-1 CODE= GL# SET = 50313
