Action History Comments are Appearing Against the Incorrect Approver in case of Consecutive Return From Corrections

Symptoms
PROBLEM STATEMENT
Action History comments are appearing against the incorrect Approver in case of consecutive Return from Corrections.
STEPS
The issue can be reproduced at will with the following steps:
1. Login to "Employee Self-Service" Responsibility.
2. Navigate to any function (say Absences, EIT, SIT etc) and enter the details and submit the transaction. Make sure the Transaction requires approval from minimum 3 Approvers in serial fashion.
3. Login as First Approver and Second Approver one after the other and approve it without entering any comments.
4. Login as final approver and Return for Correction (RFC) to Second Approver without entering any comments.
5. Login as Second Approver and Return For Correction to First Approver by entering some comments.
6. Login as First Approver and observe that the comments entered by Second Approver is not shown in Action History.
7. As First Approver, now click on Update Action and enter the details and submit the transaction. Don't have to enter any comments while submitting the Transaction.
8. Login as Second Approver and observe that the comments that were first entered by Second Approver (Self) are now showing up against Third Approver in the Action History which is incorrect.
9. Now as Second Approver , approve it without any comments.
10. Login as Final Approver and approve it without any comments.
11. Now after the transaction is completely approved, in Transaction Monitor, the comments are not visible.
