Stale Data Error Attempting to Enter PO Acknowledgements Also Acknowledgement History Showed Old/Unreleated Data

Symptoms
On : 12.2.12 version, Purchase Order
In Buyer Work Center (BWC) Order page, when the buyer tries to enter acknowledgement any Purchase Orders, the error below occurs:
Error: Stale Data
The requested page contains stale data. This error could have been caused through the use ot the browser's navigation buttons (the browser Back button, for example).
To proceed, please select the Home Ink at the top of the application page to return to the man menu. Then, access the page again using the applications navigation controls (menu, links, and so on) Instead of using the browser's navigation controls like Back and Forward
Also, in the enter acknowledgement page, the acknowledgement history section shows unrelated rows.
Steps to Reproduce:
1. Go to Purchasing Responsibility > Buyer Work Center > Orders >
2. Create new purchase order (PO)
3. Enable acknowledgement as Document
4. Submit PO for approval
5. In Buyer Work Center Order Page click Requires Acknowledge Status hyperlink
6. At Enter Acknowledgement Page, acknowledge history shows unrelated records
7. Try to enter an acknowledgement by populating any columns
8. Error above occurs
