Symptoms

While applying Consolidated upgrade patch and run the 12.2.0 Upgrade
getting following error:

 

ERROR MESSAGE

Warning: Object "AP_SUPPLIER_SITES_ALL" is not registered in table "FND_OBJECT_TABLESPACES".

AD Worker error:
The following ORACLE error:

ORA-12801: error signaled in parallel query server P00C, instance exablrdbadm02vm02.india.airtel.itm:CFINBAU2 (2)
ORA-01452: cannot CREATE UNIQUE INDEX; duplicate keys found

occurred while executing the SQL statement:

CREATE UNIQUE INDEX AP."AP_SUPPLIER_SITES_U2" ON AP.AP_SUPPLIER_SITES_ALL
 (VENDOR_ID, VENDOR_SITE_CODE, ORG_ID) LOGGING STORAGE (FREELIST GROUPS 4
 FREELISTS 4 ) PCTFREE 10 INITRANS 11 MAXTRANS 255 COMPUTE STATISTICS
 PARALLEL TABLESPACE APPS_TS_TX_IDX

Unable to compare or correct tables or indexes or keys
because of the error above

The issue can be reproduced at will with the following steps:
1. Login to the application server
2. Apply 12.2.0 upgrade patch
3. facing above warning and error message
